Make Your Decisions
Decide which insurance plans and accounts you want to enroll in. You will make a separate election for each plan during enrollment.
Make sure to review plan details ahead of time so you understand coverage levels, costs and how each plan works.
Choose your coverage level (for example, self only, self and spouse or family).
Confirm Eligibility
Make sure any dependents you plan to cover are eligible under the plan rules. Most children may be covered until age 26. Refer to Coverage Eligibility & Costs and the applicable Summary Plan Description (SPD) if you are unsure.
Add New Dependents
If enrolling a spouse, sponsored adult dependent or child for the first time, you will need their full name, birth date and Social Security Number.
Proof of Relationship
Proof of Relationship (POR) is required to enroll new dependents. After completing enrollment, submit the required documentation to the HR Service Center or your campus HR Generalist within the required time frame. See Proof or Relationship for more.
Where available, decide whether to pay your insurance premiums on a pre-tax or after-tax basis.
Premiums are deducted automatically from your paycheck and your choice affects how and when taxes are applied. Reviewing educational resources ahead of enrollment may help you decide which option is best for you.

Healthy Savings Plan
Enrollment may make you eligible for a Health Savings Account (HSA) and University-paid contributions toward healthcare savings, as well as a Health Payment Account (HPA) for a revolving line of credit for eligible expenses.

You will need to designate beneficiaries and some coverage levels may require a statement of health.
Retirement
In addition to your core retirement plan, you may choose to participate in voluntary retirement plans to supplement your long-term savings.

After you’ve made your decisions, you’ll submit your plan elections in the university’s employee portal, . You can make changes only during your enrollment period or after experiencing a qualifying family or employment status change.
Some steps, like submitting your choices and confirming your enrollment, are the same for everyone. Other requirements depend on whether you are enrolling during Annual Enrollment, enrolling for the first time, or making changes due to a life event.
